Points of interest:

all feilds in bold need to be changed to reflect your situation
Assuming you have one (1) cert representing multiple shares
On the back of the cert, in the location of the transferee's name; enter text of “see attached letter of direction”
Have every page of the letter of direction stamped with the appropriate stamp (Medallion Signature or Signature Guarantees for Securities Transfer)
Text in bold is left in bold when I send the letter in
Cert is in your name so after the TA transfers the other nine (9) you will receive a new cert for one (1) share back - no need to include yourself on the list of transfers

and hereby irrevocably constitutes and appoints TRANSFER AGENT the attorney of the undersigned to transfer the said securities with full power of substitution in this matter.
